As a Nurse Practitioner with Optum's HouseCalls program, you'll play a vital role in transforming the way healthcare is delivered. Our innovative approach focuses on providing comprehensive, in-home assessments to patients, helping to identify and address health risks before they become major issues.

Key Responsibilities:

  • Conduct in-home assessments, including medical history reviews, medication reconciliation, and comprehensive physical exams
  • Perform evidence-based practice screenings, including point-of-care testing (as needed)
  • Identify diagnoses and develop care management plans to address patient needs
  • Communicate findings with patients' primary care physicians to ensure seamless care coordination
  • Respond to urgent and emergent situations, providing timely interventions as needed
  • Educate patients on disease processes, medication management, and compliance strategies

Requirements:

  • Active NP license or ability to obtain by start date
  • ANCC or AANP national certification in Family, Geriatrics, Adult, or Adult-Gerontology Primary Care
  • Ability to complete physical demands of the job, including lifting, navigating stairs, and sitting/standing/kneeling
  • Access to reliable transportation for travel within licensed states

Preferred Qualifications:

  • 1+ years of clinical experience in a related field
  • Experience in Gerontology, Cardiology, Internal Medicine, or Endocrinology
  • Home health care or home visit experience
