Job Description

Job Summary:

Vinfen is seeking a Licensed Practical Nurse II to join our team. As a LPN II, you will provide direct service and daily nursing care in a medically intensive program designed to meet the needs of individuals with brain injuries, developmental delays, or intellectual disabilities.

Responsibilities:

  • Implement person-centered treatment care plans in accordance with medical orders.
  • Assist with activities of daily living, including showering, dressing, shaving, mouth care, foot care, and diabetes care.
  • Responsible for completing all skilled nursing care needs, including wound care, GT/JT care, trach care, and nutritional support.
  • Monitor and observe clients for any change in status and report issues in a timely manner.
  • Ensure that all care is completed using best-practice and providing the person with dignity and choice.
  • Clinically supervise C.N.A.s during designated shifts and provide in-the-moment training as needed.
  • Communicate effectively with RN Manager, primary care provider, family members, and other significant relationships.
  • Document nursing care and treatments according to Vinfen policy and procedure.

Requirements:

  • Valid Licensed Practical Nurse credentials in Massachusetts.
  • A high school diploma or equivalent is required; an Associate's degree is preferred.
  • One to two years of supervised practical experience in a clinical or rehabilitation setting is helpful, but not required.
  • Experience working with people with developmental and/or psychiatric disabilities and medical illnesses is preferred.
