We are seeking a highly skilled and compassionate Lead In-Home Therapist to join our team at WellStone. As a key member of our FIND/In-Home Intervention Program, you will work closely with families and individuals to provide mental health treatment in the community.
Key Responsibilities- Conduct intake assessments, CANS, and make referrals as identified from the assessments.
- Provide individual, family, and group therapy sessions for clients three times per week for a minimum of 12 weeks in the community.
- Visit clients in their environment to assess progress towards goals, provide support & direction.
- Advocate for clients and assist client and family members in understanding the client's diagnosis, traits, treatment approaches, etc.
- Perform crisis intervention and resolution as needed and/or required.
- Work closely with Neaves Davis Detention Center Probation Offices to provide weekly reports on clients.
- Participate in in-services, required meetings & trainings as mandated by supervisor.
- Master's Degree in Social Science
- Two years post-masters experience with SMI population preferred
- Excellent oral, written communication & planning skills
- Knowledge of community agencies and resources
- Knowledge of social security, Medicaid, & Department of Human Resources
- Excellent 5-year motor vehicle driving record
- Reliable transportation
