Lead In-Home Therapist

Job Summary

We are seeking a highly skilled and compassionate Lead In-Home Therapist to join our team at WellStone. As a key member of our FIND/In-Home Intervention Program, you will work closely with families and individuals to provide mental health treatment in the community.

Key Responsibilities
  • Conduct intake assessments, CANS, and make referrals as identified from the assessments.
  • Provide individual, family, and group therapy sessions for clients three times per week for a minimum of 12 weeks in the community.
  • Visit clients in their environment to assess progress towards goals, provide support & direction.
  • Advocate for clients and assist client and family members in understanding the client's diagnosis, traits, treatment approaches, etc.
  • Perform crisis intervention and resolution as needed and/or required.
  • Work closely with Neaves Davis Detention Center Probation Offices to provide weekly reports on clients.
  • Participate in in-services, required meetings & trainings as mandated by supervisor.
Requirements
  • Master's Degree in Social Science
  • Two years post-masters experience with SMI population preferred
  • Excellent oral, written communication & planning skills
  • Knowledge of community agencies and resources
  • Knowledge of social security, Medicaid, & Department of Human Resources
  • Excellent 5-year motor vehicle driving record
  • Reliable transportation
