We are seeking an experienced Nurse Practitioner to join our Home-based Medical Care team in Cass and Jackson Counties, MO. As a medical care professional, you will work with complex, chronic patients in their homes, providing high-end medical service and compassionate care.

Your primary responsibility will be to manage chronic illnesses in a complex elderly population with multimorbid conditions. You will also document timely, close HEDIS quality gap measures, and recapture HCCs. Additionally, you will engage new patients enrolling in the program with a comprehensive geriatric-focused initial assessment and provide treatment as needed.

Key Responsibilities

Your key responsibilities will include:

  • Managing chronic illnesses in a complex elderly population.
  • Providing timely documentation and meeting quality standards.
  • Engaging new patients and conducting comprehensive assessments.
  • Triaging, diagnosing, and treating patients on an urgent basis.
Requirements and Qualifications

To be successful in this role, you must have:

  • A completion of a Nurse Practitioner accredited program.
  • A current state NP license.
  • At least 1+ year of NP clinical experience.
  • Knowledge of CPT, ICD-9, and HCPC codes.
Compensation and Benefits

The estimated salary for this position is between $120,000 and $180,000 per year, based on experience. This includes a comprehensive benefits package, including:
