We are seeking a highly skilled and compassionate Registered Nurse (RN) to join our team at Amedisys, Inc., a leading provider of home health, hospice, and high-acuity care. As a Registered Nurse, you will play a vital role in delivering patient-centered care to individuals in their homes.
Responsibilities- Coordinate and oversee all aspects of patients' home care, ensuring seamless delivery of services.
- Collaborate with patient care teams to develop and implement comprehensive care plans, tailored to meet the unique needs of each patient.
- Educate patients and their families on their condition, needs, and care, promoting empowerment and self-management.
- Perform thorough physical assessments, checking vital signs and managing medications, as needed.
- Administer injections, IV therapy, or tube feedings, and maintain catheters, ensuring patient safety and comfort.
- Complete accurate and timely documentation, facilitating effective communication among care teams.
- Supervise Licensed Practical Nurses (LPNs) and Home Health Aides (HHAs), providing guidance and support as needed.
- Regularly communicate patient progress to the clinical manager, physician, and care team, ensuring a collaborative approach to care.
- One year of clinical experience as a Registered Nurse (RN), with a strong foundation in home health care.
- Current RN license, specific to the state(s) assigned to work.
- Minimum six months of home health experience, with a proven track record of delivering high-quality care.
- Current CPR certification, with the ability to assess patient status and identify requirements relative to age-specific needs.
- Valid driver's license, reliable transportation, and liability insurance, ensuring safe and efficient travel to patient homes.
