Mason County Skilled Nursing Facility Nurse Practitioner

At Sunbelt Staffing, we are seeking a skilled and dedicated nurse practitioner to join our team in Mason County, WA. As a Contract Nurse Practitioner, you will play a vital role in delivering high-quality care to patients in skilled nursing facilities across the county.
Salary:The estimated salary for this position is $120,000 per year, based on industry standards and location. This competitive compensation package reflects the importance of your contributions to patient care and well-being.
Job Description:As a skilled nursing facility nurse practitioner, you will be responsible for conducting comprehensive assessments, developing personalized care plans, and collaborating with interdisciplinary teams to ensure seamless care delivery. Your expertise will help improve patient outcomes, reduce hospitalizations, and enhance overall quality of life.
- Conduct one-time comprehensive assessments, including physical exams, medication reconciliation, medical history, ADLs, nutritional and cognitive assessments, pain evaluation, fall risk assessment, and social determinants of health evaluation.
- Develop and communicate problem lists and recommended treatment plans for each resident.
- Ensure accurate and complete documentation, including ICD10 coding.
- Collaborate effectively with the interdisciplinary care team.
- Facilitate advanced illness and advanced care planning conversations.
To be successful in this role, you must have:
- An active NP license in the state of practice, free of any disciplinary actions.
- A minimum of 1 year of NP experience.
- An active ANCC or AANP national certification in Family, Geriatrics, Adult, Adult-Gerontology Primary Care, or Adult-Gerontology Acute Care.
- Reliable transportation for assessments in nursing homes, assisted living, and home settings.
- DEA certification.
