Job Summary:

Liberty Health is seeking an experienced Home Care Physical Therapist to join our team. As a Home Care Physical Therapist, you will play a vital role in providing patient care services and direct treatment to patients in their homes.

Key Responsibilities:

  • Evaluate patients new to service and develop a plan of care appropriate to the patient's diagnosis.
  • Provide patient care services and direct treatment to patients in their homes.
  • Document patient care data accurately, thoroughly, and in a timely manner.
  • Communicate effectively with intercompany and intracompany contacts.
  • Discuss company and third-party reimbursement policies and guidelines with patients and families.
  • Coordinate with Patient Care Coordinators to make referrals to appropriate disciplines.
  • Provide in-service education, healthcare professional education, and public education as needed.
  • Supervise peers and support personnel as needed.

Requirements:

  • Minimum bachelor's degree in physical therapy from an accredited school of physical therapy.
  • Current licensure from the state where therapy will be provided.
  • Prefer a minimum of one year's therapeutic experience in a non-home healthcare environment.
  • Strong knowledge of theory and concepts relating to the practice of physical therapy.
  • Ability to evaluate and treat patients for whom a physician's order exists.
  • Minimum of twelve hours of continuing education annually.
