Job SummaryWe are seeking a compassionate and motivated Physical Therapist to join our team. As a Physical Therapist at JAG, you will be responsible for assessing patients' strengths and weaknesses, devising creative and effective treatment plans, and implementing a plan of care on a daily basis.
Key Responsibilities- Collect and record patient care data, including initial evaluations, daily notes, reassessments, and discharge plans.
- Prepare for evaluations, including obtaining patient history and researching unfamiliar diagnoses.
- Select and administer tests and measures, and synthesize evaluation results to formulate a diagnosis, goals, prognosis, and plan of care.
- Establish treatment plans based on evaluation results, goals, and best practice, and collaborate with interdisciplinary teams, patients, and patient support systems.
- Provide education and training to patients and their support systems, and identify the need for consultations and referrals.
- Initiate and complete discharge plans, and provide interventions to patients that are safe, effective, and in compliance with JAG standards.
- Oversee support staff, including physical therapist assistants, athletic trainers, and physical therapy aides.
- Attend annual JAG Billing and Coding seminars, and promote JAG in the community through lectures and presentations.
- Bachelor's or Master's degree in Physical Therapy, or Doctorate degree preferred.
- Licensed Physical Therapist in the state of practice.
- Completed state-regulated CEU requirements in the state of practice.
- Valid CPR license.
- Experience with EMR software preferred.
- Excellent oral and written communication skills, strong clinical decision-making skills, and a positive, professional demeanor.
- Basic computer skills and ability to work as a team member and individually with minimal supervision.
