Job SummaryWe are seeking a compassionate and experienced Hospice RN/RN Case Manager to join our team.
As a Hospice RN/RN Case Manager, you will report directly to the Administrator, Executive Director, or Patient Care Manager Senior and maintain primary control and professional management of each patient.
You will act as the primary liaison between the physician, patient/family, and hospice team, providing direct patient care services and supportive care to the patient's family.
You will empower patient caregivers by educating them on care for their loved one and manage and collaborate with an interdisciplinary group to provide comfort, peace, and dignity.
You will evaluate patient and family needs and assist in the preparation and execution of the plan of care for each patient.
You will maintain all assigned patient clinical records and participate in company quality assessment and performance improvement programs.
About YouWe are looking for a Hospice Registered Nurse/RN with 1+ years of nursing experience in hospice, home health, ICU, geriatric, or oncology.
Preferred experience includes hospital Staff RN, Med Surg, PACU, Float pool, ER or emergency room, critical care, acute care, cardiac, geriatric, admissions, wound care, telemetry, or travel nursing.
