Job Summary

We are seeking a highly skilled and experienced LPN Clinic Facilitator to join our team at Affinia Healthcare Inc. The successful candidate will provide safe and efficient care and services to patients, working under the general supervision of the Assistant VP of Health Center Operations.

Key Responsibilities
  • Assist in planning and implementation of patient-based care based on sound nursing judgment, clinical preparation, knowledge, skills, and experience.
  • Function as a member of the healthcare team, providing assistance in patient care and services.
  • Monitor and track patient information, including diagnostic test results, and provide related education and counseling.
  • Assist in the management of patient care between visits and perform specialized procedures, examinations, and/or tests as ordered.
  • Collaborate with physicians to provide health education, counseling, and advice to patients and families on topics such as lifestyle changes and diabetic controls.
  • Effectively assess patient educational needs, prioritize patient learning needs, identify barriers to learning, develop teaching plans, and use various educational tools to activate non-compliant patients.
  • Obtain and document reason for visit, patient vital signs, and assess patient/caller health status based on presenting symptoms.
  • Direct patient/caller to most appropriate level of care or an acceptable alternative venue for care of patients with acute problems.
  • Use computer knowledge to assess patient information, follow-up appointments, and perform other duties as assigned.
Requirements
  • Graduate of an accredited school of practical nursing.
  • Requires (4) four years' experience working in a community health setting, with experience coordinating clinic services.
  • Strong verbal and written communication skills.
  • Ability to triage walk-ins (emergency patients) as appropriate.
  • Computer literate.
  • Ability to coordinate the activities of a busy provider practice.
  • Valid driver's license and automobile with insurance coverage as required by the State of Missouri.
  • LPN license from the State of Missouri; CPR.
