Job Summary:

The Physical Therapist will work collaboratively with the therapy teams and Rehab Department to provide high-quality patient care. Responsibilities include completing evaluations and documentation, providing recommendations and therapy as outlined in treatment plans and prescriptions, and managing workflow to ensure timely and effective treatment.

Responsibilities:
  • Work collaboratively with the therapy teams and Rehab Department to provide high-quality patient care.
  • Complete evaluations and documentation to ensure accurate and timely treatment plans.
  • Provide recommendations and therapy as outlined in treatment plans and prescriptions to improve patient outcomes.
  • Manage workflow to ensure timely and effective treatment, including allotting time for treatment documentation and notes in the electronic records system.
  • Familiar with PDPM and align therapy treatments and plans with understanding of Medicaid and Medicare billing.
Qualifications:
  • New Jersey License as a Physical Therapist.
  • Background and experience in long-term care, specifically in a Skilled Nursing Facility.
  • Strong organizational, interpersonal, and communication skills.
  • Ability to manage patients with different types of personalities and ability levels.
