Job Summary:

The Licensed Vocational Nurse will be responsible for participating in and directing the delivery of patient care in accordance with facility clinical systems, regulatory requirements, and physician orders. You will work closely with our interdisciplinary team to ensure that residents receive the highest level of care and service.

Key Responsibilities:
  • Maintain accurate and up-to-date records of patient care and treatment plans.
  • Administer medications and treatments as prescribed by physicians.
  • Develop and implement individualized care plans that meet the unique needs of each resident.
  • Collaborate with other healthcare professionals to ensure seamless care coordination.
  • Provide education and support to residents and their families on health-related topics.
  • Participate in quality improvement initiatives to enhance patient care and satisfaction.
  • Perform other duties as assigned by management.
Requirements:
  • Current, valid licensure as a LVN required.
  • Experience in long-term healthcare preferred.
  • Strong communication and interpersonal skills necessary.
  • Ability to work effectively in a fast-paced environment.
  • Basic computer skills and proficiency in electronic medical records.
