About the Role: We are seeking a compassionate and skilled Licensed Practical Nurse to join our team in West Nyack, NY. As a member of our care delivery team, you will be an integral part of our vision to make healthcare better for everyone.

Job Summary: The Licensed Practical Nurse will provide high-quality nursing care to patients/significant others via direct and/or telephone contact, following established standards and practices. They will also collaborate with other members of the care team to ensure seamless care delivery, maximal coordination of efforts, and active patient participation in planning and care.

Key Responsibilities:

  1. Provide direct and indirect patient care services as delegated by the physician or other licensed healthcare provider
  2. Coordinate with other members of the care team to ensure effective communication and collaboration
  3. Develop and implement individualized care plans that meet the unique needs of each patient
  4. Participate in quality improvement initiatives to enhance patient care and outcomes

Requirements:

  • Current licensure as a Licensed Practical Nurse in the state of New York
  • Graduate of an accredited school of Practical Nursing
  • Ability to work collaboratively as part of a multidisciplinary care team
