About the Role

We are seeking a highly skilled and compassionate Licensed Practical Nurse (LPN) to join our team at Amedisys, a leading provider of home health, hospice, and high-acuity care. As a LPN, you will play a vital role in delivering patient-centered care to individuals in the comfort of their own homes.

Responsibilities

  • Verify the patient's plan of care with the RN prior to each visit.
  • Collaborate with the physician and RN to ensure the patient's needs are being met, and notify them promptly of any changes in the patient's condition.
  • Teach patients and caregivers self-care techniques to promote independence and well-being.
  • Check vital signs and review medications to ensure accurate and safe care.
  • Perform specialized procedures as ordered by the physician, such as foley catheter changes.
  • Prepare equipment and materials for treatments using aseptic technique.
  • Complete documentation in a timely and accurate manner.

Qualifications

  • One year of clinical LPN experience.
  • Current LPN license, specific to the state(s) you are assigned to work.
  • Current CPR certification.
  • Valid driver's license, reliable transportation, and liability insurance.
