Wraparound Facilitator

We are seeking a skilled Wraparound Facilitator to join our team at One Hope United. As a key member of our Conscience Community Network program, you will collaborate with children, families, informal and formal support systems to identify individualized needs and develop a plan of care to meet those needs.

• Convene and facilitate Family Team Meetings, ensuring the child and family voice is heard and respected by all team members.
• Develop a comprehensive plan of care, assigning responsibilities to each team member, to address needs including non-traditional services and Evidence-Based Treatments.
• Provide consultation and support to the Family Team to implement the plan of care and monitor progress toward goal achievement.
• Re-convene the team for check-ins, on agreed-upon intervals, to adjust the plan of care as needed.
• Ensure care is coordinated amongst all parties involved with the family.

• Bachelor's degree from an accredited university in psychology, social work, counseling, education, or a related field.
• Two years of experience coordinating care, facilitating family team meetings, and/or working with children and families on a direct service level.
• Strong collaborative, relationship-building, and communication skills to effectively resolve problems, achieve goals, and meet deliverables.
• Ability to work varying hours, including evenings and weekends, to accommodate family needs.

• Tuition assistance up to $2500 per year.
