Hospice Nurse Case Manager

Supreme Hospice Inc is seeking a compassionate and experienced Hospice RN Case Manager to provide end-of-life care to patients and their families.
Key Responsibilities:- Conduct thorough assessments of patients' physical, emotional, and spiritual needs
- Develop and implement individualized care plans in collaboration with interdisciplinary teams
- Provide skilled nursing care to patients in their homes or in hospice facilities
- Administer medications and treatments as prescribed by physicians
- Educate patients and families on disease processes, symptom management, and end-of-life care
- Coordinate care with other healthcare providers and community resources
- Document patient care and progress in electronic medical records
- Provide emotional support to patients and families during the end-of-life process
- Active RN license in Texas
- Minimum of 2 years of nursing experience, preferably in hospice or palliative care
- Strong assessment and critical thinking skills
- Excellent communication and interpersonal skills
- Ability to work independently and as part of a team
- Knowledge of Medicare and Medicaid regulations related to hospice care
