As a Hospice LVN, you will provide compassionate nursing care to patients in their place of residence, working under the supervision of a registered nurse. You will coordinate care with the interdisciplinary team, patient, and family, ensuring a seamless and effective care plan.
Key Responsibilities:
* Provide skilled nursing care to patients in their homes, nursing homes, and assisted living facilities
* Collaborate with the interdisciplinary team to develop and implement individualized care plans
* Conduct thorough assessments of patients' physical and emotional needs
* Develop and implement plans of care to meet patients' needs
* Educate patients and families on disease management and self-care
Requirements:
* Graduate of an accredited School of Vocational/Practical Nursing
* Current State license as a Licensed Vocational/Practical Nurse or a multi-state license issued by a state member of the National Nurse Licensure Compact (NLC)
* Two years of experience as a Licensed Vocational/Practical Nurse in a clinical care setting, hospice preferred
* Reliable transportation and valid auto liability insurance
Environmental and Working Conditions:
* Works in various environments, including patient homes, nursing homes, and assisted living facilities
* Possible exposure to blood and bodily fluids and infectious diseases
* Ability to work flexible schedule and travel locally
* Some exposure to unpleasant weather
