Patient Navigator for Obstetrics and Gynecology Department

About the RoleWe are seeking a highly skilled Patient Navigator to join our Obstetrics and Gynecology department at our Stamford location. As a key member of our team, you will play a vital role in promoting health outcomes and improving patient experiences.
Responsibilities- Develop and maintain close relationships with OBGYN patients and care team members.
- Contact patients to assist in setting up obstetric appointments and provide reminder calls prior to each appointment.
- Complete prenatal online notification forms and ensure compliance with Prenatal Measures within 14 days of the first prenatal visit.
- Assess social determinants of health and identify resources to overcome barriers to care.
- Maintain accurate patient records and track attendance at OBGYN appointments and navigation sessions.
- Resolve insurance concerns and issues by connecting patients with appropriate resources and team members.
- Provide culturally sensitive services and advocacy, patient education, and support in accessing Optimus-based, community-based, and hospital-based programs.
- Bilingual (Spanish and English) required.
- High School Diploma or GED required; Associate Degree in health and/or human services or related field preferred.
- Experience working with ethnically, culturally, and racially diverse office staff and patient population.
- Excellent interpersonal skills, including active listening, empathy, and verbal and written etiquette.
- Competitive salary: $60,000 - $80,000 per year, depending on experience.
