We are seeking a dedicated Licensed Practical Nurse (LPN) to take on the role of Evening Shift Supervisor within our esteemed Memory Care facility.
Position Overview:
The LPN Evening Shift Supervisor will play a crucial role in leading our nursing team, ensuring that our residents receive the highest standard of care. This position involves:
- Collaborating with an interdisciplinary care team to ensure comprehensive patient assessments and care planning.
- Maintaining effective communication with residents and their families.
- Overseeing the delivery of appropriate nursing care and assisting with medications and treatments.
- Supervising and mentoring nursing staff and other healthcare partners.
- Participating in daily and monthly meetings to enhance care strategies.
- Coordinating resident appointments and transportation logistics.
- Performing additional patient care responsibilities as required.
