Job Title: Nurse Practitioner

We're seeking a highly skilled and compassionate Nurse Practitioner to join our team at ChenMed. As a key member of our clinical operations team, you will be responsible for providing direct patient care to our senior patients, working collaboratively with our physicians and other healthcare professionals to ensure the highest quality of care.

Responsibilities:
  • Assess and manage acute and non-acute clinical problems in our senior patient population.
  • Develop and implement patient management plans, including ordering laboratory and diagnostic studies, prescribing medications, and referring patients for consultation when necessary.
  • Provide patient education and counseling, and facilitate patient/family conferences to review treatment goals and optimize resource utilization.
  • Collaborate with other multidisciplinary team members to analyze and evaluate current systems of healthcare delivery and identify opportunities for improvement.
  • Participate in risk and quality management programs, clinical meetings, and other meetings as required.
Requirements:
  • ARNP or similar advanced degree in Nursing required.
  • Current Nurse Practitioner Certification in the State of practice required.
  • Board certification by AANP or ANCC is preferred but may be required for certain States.
  • A minimum of 1 year of clinical experience preferred, but willing to consider strong new graduates.
  • Basic Life Support (BLS) certification from the American Heart Association (AMA) or American Red Cross required within the first 90 days of employment.
