Job Title: Case Manager

United Planning Organization (UPO) is seeking a skilled Case Manager to join our team. As a Case Manager, you will play a critical role in supporting clients in achieving their goals and improving their quality of life.

Job Summary:

The Case Manager will be responsible for assessing client needs, developing and implementing individualized plans, and coordinating access to resources and services. This is a challenging and rewarding role that requires strong communication and interpersonal skills, as well as the ability to work effectively with diverse populations.

Key Responsibilities:
  • Assess client needs and develop individualized plans to address barriers to self-sufficiency and employment
  • Coordinate access to resources and services, including emergency assistance, health and mental health services, and employment placement
  • Monitor client progress and outcomes, and make referrals to mainstream agencies as needed
  • Assist clients with housing placement and relocation, and monitor compliance with housing lease and program rules
Requirements:
  • Bachelor's degree in social work, psychology, sociology, or related field
  • 2 years of experience providing case management services, or equivalent combination of education and experience
  • Certification and/or licensure in a relevant discipline may be substituted for educational requirement
  • Knowledge of District of Columbia social service providers and community resources
  • Ability to communicate effectively orally and in writing, and to work effectively with diverse populations
Preferred Qualifications:
  • Ability to create positive, long-term working relationships by demonstrating a commitment to teamwork and customer service
  • Excellent working knowledge of MS Office and ability to use electronic time-keeping program
