Cantex is seeking a skilled Registered Nurse to join our Home Health team in a Weekend PRN position. As a key member of our team, you will be responsible for providing high-quality patient care and collaborating with our interdisciplinary team to ensure seamless care transitions.
Responsibilities- Conduct comprehensive assessments of patients' medical, social, and emotional needs
- Develop and implement individualized care plans to promote patients' potential during their home health episode
- Collaborate with physicians, clinical team members, and patients to prepare for discharge
- Supervise and manage LVN and HHA staff to ensure high-quality patient care
- Coordinate patient care services, including referrals to ancillary services and community resources
- Lead weekly interdisciplinary case conferences to discuss patient care plans and progress
- Provide individual and group support to patients and families at times of adjustment, crisis, or need
- Two years of experience in Home Health preferred
- Home Care Home Base experience preferred
- Ability to read, write, and speak English fluently
