Job Summary:

This position provides nursing support as directed by the Wellness Director to all nursing employees and Residents according to The Oaks Senior Living Policy & Procedures and to ensure regulatory compliance.

Key Responsibilities:

  • Administers and/or supervises the self-administration of medications, ensuring accurate dosing and timely delivery.
  • Verifies the identity of Residents receiving medication, confirms the correct medication is being given, and records the time of administration on the MAR/MOR.
  • Presents medication to Residents and observes ingestion or other application, or administers medication as directed.
  • Obtains and records Residents' vital signs (temperature, blood pressure, pulse, and respiration), weight, food and fluid intake and output, as directed.
  • Observes Residents to detect response to specified types of medications and prepares a report or notifies designated personnel of unexpected reactions.
  • Counsels Residents in identifying and resolving social or other problems, promoting a positive and supportive living environment.
  • Schedules doctor's appointments based on the needs of the Resident, ensuring timely and effective care.
  • Ensures that all new medications are faxed to the pharmacy in a timely manner to facilitate accurate prescription filling.
  • Informs families when mail order medicines need to be reordered, maintaining open communication and transparency.
  • Communicates with physician's offices regarding Resident changes and medication orders, ensuring seamless care coordination.
  • Ensures call lights are answered in a timely manner to attend to Residents' needs as soon as possible after the call light is signaled (7 minutes maximum).
  • Keeps accurate counts of the narcotic sheet, maintaining a secure and compliant environment.
  • Attends all training and conferences as required to stay abreast of changes in the nursing profession and maintain appropriate CEU's for licensure, with prior approval of supervisor.
  • Ensures all training required by state regulations is completed and documented each year, adhering to regulatory standards.
  • Ensures all caregiver duties are performed as assigned and documented appropriately, maintaining a high level of care and attention to detail.
  • Participates in the Care Planning process as assigned, collaborating with interdisciplinary teams to develop and implement effective care plans.
  • Assists in the oversight of appropriate training for all nursing staff to ensure requirements for state regulations are met, as assigned by supervisor.
  • Addresses employee, family, and Resident concerns to promote satisfaction and good communication, fostering a positive and supportive work environment.
  • Is fully knowledgeable of the fire evacuation and disaster policy of the community and participates as assigned in fire and disaster drills, ensuring a safe and secure environment for all.
  • Maintains confidentiality of all pertinent personal or health information concerning Residents and staff, adhering to strict confidentiality standards.

Reports to: Wellness Director/Horizons Wellness Director

Qualifications:

  • A minimum of a one-year certificate from an accredited college or technical school; three to six months related experience or training.
  • LPN license required. Management experience preferred.

Physical Job Requirements:

To fulfill job requirements, employee must be able to perform key physical tasks for extended periods of time. Employees must adhere to proper body mechanics and safety measures. In a typical eight-hour day, employee will:

1. Stand/walk up to eight hours a day.

2. Sit up to two hours a day.

3. Frequently support up to 75 pounds.

4. Occasionally lift/carry up to 50 pounds.

5. Frequently kneel, bend, and reach.

6. Secure proper assistance for transferring of Residents as needed.
