We are seeking a skilled LPN to join our team at TrustBridge, Inc. as a Hospice Nurse. As a member of our interdisciplinary team, you will play a vital role in delivering high-quality, patient-centered care to individuals and their families.
Key Responsibilities- Coordinate Patient Care: Work collaboratively with the primary physician and registered nurse to develop and implement individualized care plans.
- Identify Patient Needs: Assess the needs of patients and families, and coordinate care to meet those needs.
- Contribute to the Interdisciplinary Group: Participate in the development and implementation of individualized plans of care, and contribute to the overall care of patients.
- Provide Education and Support: Utilize teaching skills to educate patients and families on disease management, treatment options, and end-of-life care.
- Ensure Patient Comfort and Quality of Life: Utilize monitoring, intervention, and teaching skills to maximize patient comfort and enhance the quality of life for patients and their families.
- Education and Certification: Current license to practice as a Licensed Practical Nurse in the State of Florida, and current Basic Life Support Certification.
- Skills and Qualifications: Understanding of hospice nursing principles, case management, crisis intervention, communication skills, thanatology skills, and needs assessment skills.
- Language Skills: Bilingual (English/Spanish) preferred.
This is a full-time position that requires local travel to patients' homes and facilities. The ideal candidate will be able to work collaboratively as part of a team, and will possess excellent communication and interpersonal skills.
