As a Licensed Practical Nurse (LPN) at Country Meadows of Frederick, you will play a vital role in fostering a supportive and nurturing atmosphere for both residents and colleagues. Your expertise in patient care will allow you to build meaningful relationships, utilize your assessment skills, and make informed decisions to manage care effectively.
Work Schedule: Full-time position available from 3pm to 11pm.

Key Responsibilities:
- Develop strong relationships with residents, their families, and healthcare providers to create effective care plans.
- Guide and support colleagues in monitoring and addressing residents' needs promptly.
- Collaborate with physicians to oversee care orders and medication management, ensuring high-quality care is delivered consistently.
- Provide education to residents and their families on health and wellness topics relevant to their care.
- Assume the role of Manager on Duty as needed.
Qualifications:
- Valid LPN license in the state of employment.
- Current CPR certification; we will cover the cost if certification is needed.
- At least one year of experience in assisted living or a similar healthcare environment.
- Strong leadership abilities and relationship-building skills with residents and team members.
- Capability to perform the physical tasks associated with this role.
