Job Summary:
As a key member of our team, the Surgical Care Coordinator RN will play a vital role in ensuring seamless care coordination for surgical patients. This role requires strong clinical expertise, excellent communication skills, and the ability to work collaboratively with various stakeholders. The ideal candidate will have a strong background in nursing, with a minimum of two years of direct patient care experience.

Key Responsibilities:
  • Develop and implement comprehensive care plans for surgical patients, ensuring timely and effective coordination of care.
  • Collaborate with multidisciplinary teams to identify and address patient needs, ensuring seamless transitions across the care continuum.
  • Provide education and support to patients, families, and caregivers, empowering them to make informed decisions about their care.
  • Monitor and analyze data to identify trends and areas for improvement, driving quality and efficiency initiatives.
  • Stay up-to-date with the latest clinical guidelines, protocols, and best practices, applying this knowledge to optimize patient outcomes.

Requirements:
  • Registered Nurse License (Hawaii) required at hire.
  • Basic Life Support certification from the American Heart Association.
  • Strong communication and interpersonal skills, with the ability to work effectively in a fast-paced environment.
  • Ability to work independently and as part of a team, with a strong focus on patient-centered care.
