Position Overview:
This role requires an active FL/Compact nursing license. The Registered Nurse Case Manager will be responsible for managing a caseload of adults and geriatrics with a patient ratio of 1:25-30.
Work Schedule:
The position involves a 5x8 hour workweek, from 8:30 AM to 5:00 PM, Monday through Friday, with one full weekend commitment every four weeks. There are no on-call duties or holiday shifts required.
Responsibilities:
The Case Manager will work closely with an in-house team to ensure that all necessary diagnostic tests, consultations, laboratory work, and procedures are completed efficiently and effectively. The average daily caseload is between 25-30 patients during weekdays, with variations on weekends focused on observation patients and facilitating safe discharges.
Qualifications:
- Minimum of 3 years of Case Management experience in an acute care hospital setting.
- Proficiency in Cerner, CarePort (Allscripts), and Powerchart applications.
- Familiarity with Medicare Conditions of Participation documentation.
- Ability to thrive in a fast-paced, high-volume environment.
- Level 1 Trauma experience is preferred.
- Candidates must possess a degree from an accredited institution or one approved by the FBON, along with 2 years of recent acute care experience, which must be consecutive within the last 24 months in a US hospital.
