POSITION SUMMARY :

The Chronic Care Case Manager plays a vital role in the healthcare team, utilizing a collaborative process of assessment, planning, facilitation, and advocacy to meet the health needs of Medicare patients.

The primary function of this position is to assist Medicare patients in the administration of their health plan benefits, focusing on arranging or coordinating services that the patient needs to get well or stay well.

The Chronic Care Case Manager demonstrates strengths working independently as well as collaboratively within a multi-specialty healthcare environment, rotating among several clinics to perform non-face-to-face encounters with the clinic's Medicare population.

The ideal candidate will possess a graduate degree in nursing, current state licensure, and at least three years of nursing experience. Knowledge of EMR, medical management, and policies and procedures is also required.

QUALIFICATIONS:
Graduate of accredited school of nursing required-Registered Nurse, with current state licensure required. Prefer at least three years nursing experience. Knowledge of EMR, medical management and policies and procedures. Intermediate skills in word processing, spreadsheet and database development and maintenance. Excellent organizational, written, and oral communication skills.

WORK SCHEDULE:
Monday-Friday 8:00am-5:00pm
