Job Summary

We are seeking a skilled Physical Therapist to join our Home Health team. As a Physical Therapist, you will evaluate and develop treatment plans for patients of all ages, modifying the plan of care as appropriate.

You will serve as a role model in the delivery of professional services and as a clinical resource for staff and students. Hours may vary depending upon census and program need.

Responsibilities
  • Evaluate and develop treatment plans for patients of all ages
  • Modify the plan of care as appropriate
  • Serve as a role model in the delivery of professional services
  • Provide clinical resource for staff and students
  • Administer treatment involving application of physical agents such as heat, cold, light, water and electricity or traction and massage therapy
  • Evaluate, fit and adjust prosthetic and orthotic devices and recommend modifications to the orthotist/prosthetist
  • Plan and administer prescribed physical therapy treatment programs for patients to restore function, relieve pain and prevent disability, injury or loss of body part
  • Perform patient assessment tests, measurements and evaluations, such as: range of motion and manual muscle tests, gait and functional analyses, and body part measurements
  • Record and evaluate findings to aid in establishing or revising specifics of treatment programs
  • Review and evaluate physician or podiatrist referral and patient medical record to determine if physical therapy required
  • Demonstrate comprehension and adherence to regulatory requirements and home care policies regarding practice, and documentation content and timelines
  • Complete functional assessments and documentation per regulations
  • Complete comprehensive OASIS assessment of patients
  • Assess homebound status of patients
  • Complete comprehensive physical therapy assessment in patient's home and develop plan of care
  • Communicate with patients to schedule appointment times and manage daily schedule
  • Assess home safety and recommend equipment to improve patient's home safety
  • Educate family/caregivers on recommended home exercise program
  • Perform wound care within the scope of practice and according to the plan of care and to the department policy
Requirements
  • Bachelor's Degree in Physical Therapy
  • Physical Therapist (PT) License Illinois upon hire and Driver's License upon hire and Proof of Auto Insurance upon hire
  • One (1) year Healthcare/Medical - Physical Therapist (preferred)
  • Ability to work as a member of an interdisciplinary team
  • Drives reliable transportation and maintains state required insurance and valid Illinois Drivers License
