Job Title: Licensed Practical Nurse

We are seeking a skilled Licensed Practical Nurse (LPN) to join our Home Health team at Adventist HealthCare. As an LPN, you will play a vital role in providing high-quality patient care and contributing to the success of our team.

Job Summary:

The Licensed Practical Nurse (LPN) will be responsible for providing skilled nursing care to patients in their homes, focusing on promoting health, preventing illness, and alleviating suffering. This role requires strong communication and interpersonal skills, as well as the ability to work independently and as part of a team.

Key Responsibilities:
  • Conduct thorough patient assessments to identify care needs and develop individualized care plans.
  • Provide skilled nursing care, including wound care, medication management, and vital sign monitoring.
  • Collaborate with the interdisciplinary team to ensure comprehensive care and coordinate services.
  • Communicate effectively with patients, families, and healthcare providers to ensure seamless care transitions.
  • Document patient care and progress in a timely and accurate manner.
Requirements:
  • Valid LPN license in Maryland.
  • Minimum of three years' experience in Med/Surg nursing, with at least two years in a supervisory setting.
  • Valid Driver's License with agency insurance requirements.
  • Active American Heart Association Basic Life Support (BLS) certification.
Work Schedule:

Monday through Friday, days with holiday rotation, occasional weekends.
