Key Responsibilities- Conduct thorough assessments of patient needs and develop personalized care plans to achieve optimal clinical outcomes.
- Collaborate with physicians, referral sources, and other healthcare professionals to ensure seamless care coordination.
- Lead and coordinate care teams to ensure timely and effective delivery of care services.
- Monitor patient progress and adjust care plans as needed to ensure the best possible outcomes.
- Stay up-to-date with the latest clinical policies, procedures, and regulations to ensure compliance and quality care.
- Bachelor of Science degree in Nursing (BSN) preferred.
- At least 2 years of clinical home care experience, with a minimum of 1 year in a clinical management role.
- Current CPR certification.
- Excellent verbal and written communication skills.
- Ability to work independently and as part of a team.
- Knowledge of state and federal home health agency regulations and compliance standards.
