About the Role:
Neighborcare Health is seeking a skilled Registered Nurse to join our team in the outpatient clinic setting. As a key member of our interdisciplinary team, you will work in partnership and joint accountability to achieve our mission and goals.
Key Responsibilities:
- Provide high-quality patient care in the outpatient clinic setting, working collaboratively with colleagues to meet the needs of patients.
- Maintain joint accountability to the clinic team and medical program, supporting colleagues as needed.
- Work with team members and leaders to define and reassess priorities, plan, communicate, and execute care delivery.
- Participate in Quality and Performance Improvement efforts to improve patient outcomes.
- May serve as a preceptor for new staff and/or students assigned to Neighborcare as part of their professional education.
- Provide community outreach to people experiencing homelessness in collaboration with the clinic team and community partners.
Chronic Condition Management:
Establish partnerships with patients and families to improve health status, identifying patient goals and assessing understanding of their condition.
Provide education on disease processes, offer tools and skills for self-management, and monitor, track, and follow-up with patients to ensure care needs are met.
Coordinate care with outside providers and resources, applying Neighborcare Health guidelines and standing orders.
Offer medication-assisted treatment (MAT) to clients experiencing opioid use disorder and other substance use disorders, following a harm reduction model and using motivational interviewing.
Patient Education:
Provide patient education across the lifespan, assessing barriers to learning and tailoring education to meet individual needs.
Educate patients about medications, abnormal laboratory results, and self-management of acute and chronic conditions, promoting wellness and preventive care.
Provide motivational interviewing and harm reduction counseling, triaging emergent health issues and performing select clinical procedures as needed.
Requirements:
- Completion of an accredited RN program
- Valid Washington State Registered Nurse license
- BLS certificate
- Preferred qualifications: 1+ years RN experience in primary care, hospital, and/or home care setting or comparable social service setting, Board Certification in applicable specialty areas

Compensation:
The wage range for this position is a minimum $34.17/hour to $47.41/hour maximum, determined based on years of relevant experience.
