The estimated salary for this position ranges from $63,370 to $74,505 per year, depending on experience and qualifications. Job Description

As a Hospice Registered Nurse/RN Case Manager with Gentiva, you will have the opportunity to make a meaningful difference in the lives of patients and their families. Your primary responsibilities will include:

  • Providing direct patient care services and supportive care to patients' families
  • Acting as a liaison between physicians, patients, and the hospice team
  • Evaluating patient and family needs and developing personalized care plans
  • Maintaining accurate and timely clinical records
Required Skills and Qualifications

To be successful in this role, you will need:

  • A valid RN license in the state where employment is requested (or eligibility to obtain)
  • At least one year of experience in hospice, home health, ICU, geriatric, or oncology nursing
  • Excellent communication and interpersonal skills
  • Ability to work independently and collaboratively as part of a multidisciplinary team
