We are seeking a highly motivated and experienced Registered Nurse Case Manager to join our team at Focus Staff. As a Traveling RN Case Manager, you will be responsible for providing case management services to patients in a hospital setting.
Responsibilities:- Provide case management services to patients according to policy and procedure.
- Communicate effectively with patients, families, and healthcare teams to ensure seamless care coordination.
- Assess patient needs and develop individualized care plans to achieve optimal outcomes.
- Collaborate with healthcare providers to ensure timely and effective care delivery.
- Monitor patient progress and adjust care plans as needed to ensure optimal outcomes.
- Current state license required.
- B.S. in Nursing or related health field preferred but not required.
- Minimum of three years hospital-based nursing practice with experience in utilization/case management.
- BLS for Healthcare Providers required within 30 days of hire.
