Responsibilities in Medical Coordination
1. Provide coverage for Direct Care staff during shortages.
2. Conduct weekly meetings with the Registered Nurse and arrange necessary medical, dental, psychiatric, and other clinical appointments for consumers.
3. Accompany consumers to their medical appointments within the community.
4. Ensure consumers are properly dressed and groomed before leaving the residence during duty hours.
5. Administer consumer medications as prescribed and maintain accurate Medication Administration Records.
6. Ensure that lunches and snacks brought into the community align with consumer dietary menus.
7. Adhere to additional duties assigned by the Manager or Director.
8. Meet with the Dietician monthly to ensure consumer nutritional requirements are fulfilled.
Direct Care Duties
Oversee the upkeep of the physical environment through daily maintenance checks.
Ensure weekly monitoring of consumer body weights and maintain relevant data.
Facilitate the achievement of residents' daily goals and ensure proper data maintenance.
Implement behavioral goals and maintain accurate data records.
Ensure the maintenance of residence vehicles and document travel logs.
Research recreational activity options and assist in the direct care of consumers as necessary.
Supervise on-shift staff and provide disciplinary recommendations to the Residence Manager when needed.
Engage in various projects as assigned by the Manager or Director.
Qualifications: Excellent communication skills are essential. A High School Diploma is required. An unrestricted New York State Driver's License is mandatory.
