Job Title: Nurse Practitioner - Advanced Practice Provider

Oak Street Health is seeking a highly skilled and compassionate Nurse Practitioner to join our team in Tyler, TX. As an Advanced Practice Provider, you will play a vital role in delivering high-quality, patient-centered care to our Medicare patients.

Job Summary:

We are looking for a dedicated and experienced Nurse Practitioner to provide primary care services to our patients. The ideal candidate will have a strong background in family medicine, gerontology, or internal medicine, and will be able to work independently and collaboratively as part of our interdisciplinary care team.

Responsibilities:
  • Provide comprehensive primary care services to patients, including routine check-ups, acute care, and chronic disease management.
  • Conduct thorough health risk assessments and develop personalized care plans to address patients' unique needs.
  • Collaborate with our care team, including physicians, nurse practitioners, and medical assistants, to ensure seamless care coordination and communication.
  • Participate in quality improvement initiatives and contribute to the development of best practices in primary care.
  • Stay up-to-date with the latest medical research and guidelines, and apply this knowledge to improve patient outcomes.
Requirements:
  • Master's degree in Nursing or Doctor of Nursing Practice (DNP) degree.
  • Current licensure as a Nurse Practitioner in the state of Texas.
  • Board certification in a relevant specialty (e.g., family medicine, gerontology, internal medicine).
  • At least 2 years of experience in primary care, preferably in a community health setting.
  • Excellent communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare teams.
