About the Role:
CenterWell Home Health is seeking a skilled Occupational Therapist to join our team. As a Home Health Occupational Therapist, you will work with patients in their homes to help them recover from injury, illness, or surgery.
Key Responsibilities:
- Assess patients' daily living and work-related skills and develop therapeutic retraining programs.
- Administer and interpret diagnostic and prognostic tests to assist physicians in evaluating patients' level of function.
- Confer with physicians and other healthcare team members to develop and revise treatment plans.
- Provide therapeutic treatment and instruction to patients to improve strength, coordination, range-of-motion, and function.
- Review and expand retraining programs to meet patients' behavioral goals and rehabilitation program guidelines.
- Consult with vocational team members to develop and implement vocationally oriented plans.
- Report patient status and progress to physicians, rehabilitation staff, Clinical Manager, patients, and family members.
- Participate in care coordination and discharge planning activities and act as a resource to other healthcare personnel.
- Design community reintegration activities to assist clients in physical reconditioning and psychological adjustment.
- Recommend and design special adaptive equipment to improve residence or working environments.
- Design and train staff and family members to carry out retraining programs.
- Coordinate and oversee Occupational Therapy Assistant and Home Health Aide performance in implementing occupational therapy services.
- Accurately document patients' care observations, interventions, and evaluation.
Requirements:
- Degree from an accredited Occupational Therapy Program.
- A minimum of six months of occupational therapy experience preferred.
- Home Health experience a plus.
- Current and unrestricted OT licensure.
- Current CPR certification.
- Good organizational and communication skills.
- A valid driver's license, auto insurance, and reliable transportation are required.
