2. Facilitate ongoing care for patients with chronic conditions by:
a. Conducting thorough health assessments.
b. Performing medication reconciliations.
c. Implementing motivational interviewing techniques and self-management goal setting.
d. Providing comprehensive patient education.
e. Referring patients to appropriate services and resources.
3. Support the transition of patients from inpatient to outpatient care by:
a. Assessing transitional needs.
b. Reviewing and establishing contingency plans.
c. Assisting with post-discharge requirements such as prescriptions and follow-up appointments.
4. Coordinate with healthcare providers to create or update patient care plans.
5. Ensure accurate and timely documentation in electronic medical records.
6. Participate in daily team meetings and patient care coordination discussions.
7. Maintain ongoing professional development and stay informed about current evidence-based practices.
Qualifications:
- Bachelor's degree in Nursing or an Associate's degree in Nursing with a minimum of 4 years of RN experience.
- Active and unrestricted RN license in the state of employment.
- Current BLS certification.
- At least 2 years of experience in a clinical or hospital environment.
- Knowledge of chronic illnesses, particularly COPD, diabetes, CHF, and IHD.
- Proficient in using electronic medical records.
- Strong communication skills and ability to work effectively within multidisciplinary teams.
- Excellent organizational and prioritization abilities.
Preferred Qualifications:
- Experience in patient education and motivational interviewing.
- Bilingual in English and Spanish.
Physical and Mental Requirements:
- Ability to lift and move objects weighing up to 35 pounds.
- Capacity to sit or stand for extended periods.
- Proficient in fine motor skills for operating office equipment.
- Ability to comprehend and follow verbal and written instructions.
- Strong problem-solving skills for both simple and complex situations.
