Job Description & Requirements: We are seeking an experienced Nurse to join our team as a Home Health Registered Nurse (RN). The successful candidate will be responsible for providing skilled care and case management to patients/families in accordance with the State Nurse Practice Act and accepted professional standards of practice.

Responsibilities:
  • Assessing patient needs and developing individualized care plans
  • Providing direct care and teaching to patients and families
  • Collaborating with multidisciplinary teams to ensure seamless care delivery
  • Coordinating services and resources to meet patient needs

Requirements:
  • Graduate of an approved, accredited school of professional nursing required
  • Current license to practice as a Registered Nurse in the Commonwealth of Massachusetts required
  • Minimum of one (1) year Medical/Surgical nursing experience required
  • Demonstrated excellent verbal and written communication skills and strong interpersonal skills required
