Pharmaceutical Care Specialist

Job Opportunity:
Jamaica Hospital Medical Center, a renowned Queens-based hospital and Level 1 Treatment Center, is seeking an experienced Clinical Pharmacy Specialist to join our team. As a Clinical Pharmacy Specialist, you will provide clinical pharmacy services for inpatient and outpatient hematology/oncology medication management.
The ideal candidate will have a Doctor of Pharmacy degree from an accredited College of Pharmacy or equivalent, with Board Certification in Pharmacotherapy (BCPS) or Board-Certified Oncology Pharmacist (BCOP). A completion of PGY1 Pharmacy Practice residency and 3-6 years oncology experience is preferred. PGY2 Specialty residency in Adult Oncology or equivalent Adult Oncology experience and minimum 2 years of clinical experience is also desirable.
About the Role:
- Collaborate with inter-professional teams to enhance communication, coordinate care, and improve patient satisfaction.
- Evaluate drug therapy for appropriate indications, dosage, drug interactions, and drug allergies, monitoring patients' pharmacotherapeutic regimens for effectiveness and adverse drug events (ADEs).
- Identify ADEs and assist in their management and prevention, reporting process improvements to reduce drug errors and preventable ADEs.
Responsibilities:
- Provide pharmacokinetic monitoring when targeted drugs are prescribed.
- Coordinate with stakeholders to ensure timely access to medications.
- Partner with patients to determine the most suitable sources for their medication needs and refer patients to relevant programs and resources.
- Assist with insurance-related issues and collaborate with coordinators of prior medication authorizations.
