We are seeking a compassionate and skilled nurse practitioner to join our team at Abode Care Partners in Cincinnati, OH.
Key Responsibilities- Provide evidence-based medical care to patients in their homes, assisted living facilities, or independent living communities.
- Examine and treat chronic and urgent episodic illnesses, minor injuries, accidents, and other injuries or illnesses.
- Order and interpret laboratory and diagnostic testing as needed.
- Collaborate with caregivers and other healthcare professionals to provide interdisciplinary care and maintain a comprehensive plan of care.
- Ensure patient medical records are kept up to date.
- Active state Nurse Practitioners license.
- Current Nurse Practitioner National Board Certification.
- Active state DEA licensure.
- Master's degree in Nursing (MSN).
- Five or more years of nursing experience; at least two years of nurse practitioner experience preferred.
- Total annual compensation may range between $105,000 to $120,000 based on performance.
- Quarterly opportunities to increase base salary along the way.
