Key Responsibilities- Assessment and Care Planning: Conduct ongoing physical and psychosocial assessments of patients to identify their needs and develop individualized care plans.
- Medication Administration: Prepare and administer medications as prescribed by physicians and document patient responses.
- Patient Education: Educate patients and their families on disease processes, treatment options, and self-care techniques.
- Communication: Communicate effectively with patients, families, and healthcare team members to ensure seamless care coordination.
- Documentation: Accurately document patient information, including vital signs, medications, and treatment plans.
- Education: Graduation from an accredited school of Practical Nursing and licensure to practice as a Practical Nurse in the State of New Jersey.
- Experience: Minimum one-year adult or pediatric medical/surgical clinical experience.
- Skills: Strong nursing assessment and documentation skills, clinical knowledge to support the dying process, and excellent interpersonal and communication skills.
As a Per Diem Hospice LPN at the Karen Ann Quinlan Memorial Foundation, you will have the opportunity to make a meaningful difference in the lives of our patients and their families. We offer a competitive compensation package, including a starting rate of $29.00 per hour, and opportunities for professional growth and development.
