RNAC Coordinator

At St. Martha Center, we are seeking a highly skilled Registered Nurse Assessment Coordinator (RNAC) to join our team. As a RNAC, you will play a critical role in ensuring the highest level of quality care for our residents.

• Review current Medicare charts to ensure documentation compliance with Medicare regulations.
• Coordinate the Plan of Care with the clinical team to ensure seamless care delivery.
• Ensure pain assessments and care plans are addressed to meet the needs of our residents.
• Conduct utilization reviews to ensure accurate Medicare certifications and hospital referrals.
• Collaborate with other departments to ensure accurate MDS coding and data entry.
• Stay up-to-date with the latest MDS coding updates and changes to ensure compliance.
• Develop and maintain a good working relationship with interdepartmental personnel to ensure effective communication.
• Participate in weekly CMI meetings to discuss resident care and quality improvement initiatives.

• Current RN license in the Commonwealth of Pennsylvania.
• RNAC Certification.
• Experience in MDS coding and Medicare compliance.
