We are seeking a skilled and compassionate Home Health Registered Nurse to join our team at CenterWell Primary Care. As a Home Health Registered Nurse, you will play a vital role in providing high-quality patient care and support to individuals in their homes.

Job Summary

The successful candidate will be responsible for providing admission, case management, and follow-up skilled nursing visits for home health patients. You will work closely with our interdisciplinary team to develop and implement individualized care plans that meet the unique needs of each patient.

Key Responsibilities
  • Provide admission, case management, and follow-up skilled nursing visits for home health patients.
  • Develop and implement individualized care plans in collaboration with the patient, family, and healthcare team.
  • Confer with physicians to develop and revise care plans as needed.
  • Coordinate care with other healthcare professionals, including physical therapists, occupational therapists, and home health aides.
  • Monitor patient progress and report to the patient's physician and Clinical Manager.
  • Discharge patients after consultation with the physician and Clinical Manager.
Requirements
  • Diploma, Associate, or Bachelor's Degree in Nursing.
  • Minimum of one year nursing experience, preferably in home health or a related field.
  • Strong med-surg, ICU, ER, and acute experience.
  • Current and unrestricted Registered Nurse licensure.
  • Current CPR certification.
  • Valid driver's license, auto insurance, and reliable transportation.
