Responsibilities
  • Provide comprehensive physical therapy services, including assessment, evaluation, and treatment planning, to patients in their homes.
  • Develop and implement individualized care plans to meet the unique needs of each patient.
  • Communicate effectively with patients, families, and healthcare teams to ensure seamless care coordination.
  • Supervise and instruct non-professional home health aide staff in the provision of personal care to patients.
  • Stay up-to-date with the latest clinical guidelines and best practices in physical therapy.
Requirements
  • A current, unencumbered State Physical Therapy License in the state of practice.
  • One year of experience as a home care professional Physical Therapist, with a strong background in comprehensive assessment and OASIS-C.
  • Excellent communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare teams.
  • Proficiency in Microsoft Office and other relevant software applications.
  • A valid driver's license, good driving record, and reliable mode of transportation.
