Key Responsibilities:
- Conduct social evaluations, assessing family dynamics, caregiver capabilities, communication patterns, and identifying risks for suicide, neglect, or abuse. Develop intervention plans based on evaluation outcomes.
- Evaluate special needs related to the cultural backgrounds of patients and their families.
- Maintain comprehensive clinical records for all patients referred to social work.
- Engage in the creation of individualized care plans in collaboration with patients and families, and participate in regularly scheduled interdisciplinary meetings.
- Assist physicians and team members in understanding significant social and emotional factors affecting health issues and end-of-life concerns.
- Support families and patients in making arrangements for funerals, as well as financial, legal, and healthcare decision-making responsibilities.
- Ensure adherence to all state and federal legal and regulatory standards.
Qualifications:
- Master's degree in social work from a program accredited by the Council on Social Work Education.
- Current licensure to practice in accordance with Medicare guidelines in the state of operation.
- A minimum of one year of experience in Health Care Social Work, preferably in Home Health or Hospice settings.
