Position Overview:

As a Registered Nurse Case Manager at Gentiva Home Hospice, located in Sandwich, MA, we view each day as a chance to personalize care and enhance comfort.

We offer tailored personal care, palliative care, and hospice services to ensure individuals receive the dignified, graceful, and loving care they deserve.

Our team visits patients in their residences or facilities, wherever they consider 'home'. This is a Full-Time position with comprehensive benefits, operating Monday through Friday during daytime hours (e.g., 8 AM - 5 PM).

Flexibility is essential to accommodate patient and organizational needs. A valid driver's license and automobile insurance are required, along with current CPR certification.


As a Hospice RN Case Manager, your responsibilities will include:

  • Reporting directly to the Administrator, Executive Director, or Patient Care Manager Senior while maintaining primary oversight and professional management of each patient.
  • Serving as the main point of contact between the physician, patient/family, and hospice team.
  • Delivering direct patient care services to hospice patients and providing supportive care to their families while managing a reasonable caseload and fostering a trusting, compassionate relationship.
  • Empowering caregivers by educating them on the care of their loved ones.
  • Collaborating with an interdisciplinary team that includes Licensed Practical/Vocational Nurses (LPN or LVN), Certified Nursing Assistants (CNA), and support staff to ensure comfort, peace, and dignity.
  • Assessing patient and family needs and assisting in the development and implementation of individualized care plans.
  • Maintaining all assigned patient clinical records.
  • Participating in company quality assessment and performance improvement initiatives.

About You:
As a Hospice Registered Nurse / RN, you should possess a compassionate heart, demonstrating kindness and empathy towards patients and their families.

Case Manager Qualifications:
  • A genuine commitment to providing care in the patient's chosen environment.
  • The ability to remain flexible and composed while supporting others during challenging times.
  • Possession of an RN license in the state of employment (or eligibility to obtain one).
  • A minimum of 1 year of nursing experience in hospice, home health, ICU, geriatric, or oncology settings is strongly preferred.

Additional preferred experience that may contribute to your success in a hospice role includes:

  • Hospital Staff RN, Med Surg, PACU, Float Pool, ER, critical care, acute care, cardiac, geriatric, admissions, wound care, telemetry, or travel nursing.

A valid driver's license and automobile insurance are required, along with current CPR certification. Gentiva is a family of industry-leading hospice, palliative, and personal care providers.
