Key Responsibilities:
As an Ambulatory Clinical Pharmacist, your expertise will be essential in the following areas:
1. Implementing pharmacy services and clinical programs in alignment with organizational policies.
2. Conducting thorough reviews of patient medication records to assess therapy appropriateness and identify candidates for outreach.
3. Evaluating patient conditions and addressing barriers to medication adherence, ensuring comprehensive treatment of health issues.
4. Recognizing untreated health concerns and facilitating referrals to appropriate healthcare professionals.
5. Developing medication management plans that prioritize patient safety and minimize adverse effects.
6. Providing healthcare teams with essential information regarding medication therapies to support safe prescribing.
7. Collaborating with medical staff to ensure the highest standards of patient care.
8. Advising on the proper handling, storage, and administration of medications in the clinical environment.
9. Prescribing, adjusting, and discontinuing medications in accordance with established collaborative practice agreements.
10. Maintaining accurate documentation of medication plans and patient progress to support effective care and billing practices.
11. Engaging in quality improvement initiatives through data collection and analysis to enhance service delivery.
Qualifications:
Minimum:
- PharmD Degree
- At least two years of experience in a pharmacy retail or ambulatory setting, or completion of a pharmacy residency program.
Preferred:
- Experience in managing cardiology-related conditions.
- Strong interpersonal skills for effective collaboration with providers, patients, and clinic staff.
- Ability to apply critical thinking and creativity in problem-solving.
Physical Requirements:
Ability to perform tasks such as hearing, lifting, manual dexterity, and mobility.
