Job Summary

We are seeking a skilled Palliative Care Nurse Practitioner to join our team. As a Palliative Care Nurse Practitioner, you will play a vital role in providing compassionate and comprehensive care to patients and their families, focusing on symptom management, pain control, and quality of life.

Responsibilities
  • Provide high-quality, patient-centered care to patients with serious illnesses, focusing on symptom management and pain control.
  • Collaborate with the interdisciplinary team to develop and implement individualized care plans.
  • Conduct thorough assessments, develop and implement treatment plans, and provide ongoing care and support to patients and their families.
  • Communicate effectively with patients, families, and healthcare teams to ensure seamless care coordination.
  • Participate in quality improvement initiatives and contribute to the development of policies and procedures.
Requirements
  • Graduated from an approved school of professional nursing.
  • One year experience as a Nurse Practitioner is preferred.
  • Licensed to practice as a Nurse Practitioner in the state of agency operation.
  • Active DEA license in the state where care is being provided.
  • Possess and maintain valid CPR certification while employed in a clinical role, preferred.
  • Must be a licensed driver who can travel to all business locations.
