Key Responsibilities- Conduct clinical assessments on prospective residents and day one admission nursing observation assessments.
- Complete required admission paperwork with new residents and family.
- Participate in interdisciplinary team (IDT) process.
- Assist residents with adjustment to their new surroundings.
- Cultivate relationships with discharge planners across different health systems.
- Track admissions data with follow-up.
- Assist with discharge process.
- Ensure all admission process and checklist items are completed.
- Current RN license.
- 1+ year experience in skilled nursing required.
- Knowledge of local, state, and federal guidelines related to long-term care and resident rights.
- Ability to learn multiple databases.
- Excellent customer service and communication skills.
